Cancellation Policy

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the therapists’ day that could have been filled by another patient. As such, we require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be charged the total cost of the visit or a session in their package unless otherwise stated. This fee must be paid before a new appointment is scheduled and will be charged to the patient’s payment account on file. Providing updates to all contact information are the sole responsibility of the patient.

Consent for Chiropractic Treatment and Care

If chiropractic treatment is deemed necessary, I hereby request consent to the performance of chiropractic adjustments and other chiropractic/ physical therapy procedures, including various modes of physiotherapy (or on the patient named below, for whom I am legally responsible for) by the doctor or intern, affiliated with Deluca Mull Chiropractic Kinetic Impact Rehab and Performance, Inc.

I understand that, as in the practice of medicine, in the practice of chiropractic and physical therapy care there are some risks to treatment including, but not limited to, fractures, disc injuries, strokes, dislocations and sprains. I do not expect the doctor to be able to anticipate and explain all risks and complications.

Financial Policy

Financial Policy (Please disregard if you are a Veteran covered by TriWest)

Below is our fee schedule for our services that we provide:

60 Min Session (Single ) $175

60 Min Session (10 Pack) $1650

60 Min Session (20 Pack) $3000

Payment for treatment/coaching: You agree to pay by cash, check, or credit card on the day that treatment is rendered. Unless we approve other arrangements in writing, the balance on your account is due and payable when the services are rendered and is past due if not paid by this time.

Missed Appointment Fees: Clients who do not show up for a 1-on-1 or semi-private coaching session appointment or cancel with less than 24 hours’ notice will be charged the total cost of the visit unless otherwise stated. This fee must be paid before a new appointment is scheduled and will be charged to the patient’s payment account on file. Providing updates to all contact information are the sole responsibility of the client.

Returned Checks: There is a fee (currently $40) for any checks returned by the bank.

Refunds for any sessions unused will be returned within 5 business days upon request.
